Course Overview

This CME presentation provides a practical, evidence-based approach to the diagnosis of venous thromboembolism (VTE), including deep vein thrombosis (DVT) and pulmonary embolism (PE), with an emphasis on appropriate use of clinical prediction tools, D-dimer testing, and diagnostic imaging. The presentation also addresses VTE diagnosis in pregnancy, upper-extremity DVT, and strategies to reduce unnecessary imaging while maintaining diagnostic safety.

The course further explores hereditary thrombophilias, including Factor V Leiden, prothrombin gene mutation, and deficiencies of antithrombin, protein C, and protein S. Participants will examine when thrombophilia testing is appropriate, the potential benefits and limitations of testing, factors that may affect test accuracy, and how results may influence anticoagulation and clinical decision-making.

Objectives

  • Apply an evidence-based, algorithmic approach to the diagnosis of deep vein thrombosis (DVT) and pulmonary embolism (PE).

  • Utilize clinical prediction rules and D-dimer testing to assess pretest probability and determine the appropriate need for diagnostic imaging in suspected VTE.

  • Apply pregnancy-specific diagnostic strategies for suspected pulmonary embolism while considering the appropriate use of laboratory testing and imaging.

  • Identify patients for whom hereditary thrombophilia testing may be appropriate and recognize factors that can affect the accuracy and clinical utility of testing.

  • Evaluate thrombophilia results and patient-specific risk factors to support informed decisions regarding anticoagulation duration and prevention of recurrent VTE.
